What the Immunization and Physical Compliance Form asks for

The Immunization and Physical Compliance Form is a College of Nursing document, named in the Clinical Compliance Policy of the 2026-2027 Kent State catalog. It gathers two kinds of health proof in one place: the vaccines you have received and the immune-status tests, usually called titers, that show you are protected. A licensed provider signs it, so the form carries a clinician's confirmation rather than your own word.

The catalog names the form without printing every vaccine and titer on it. Work from the current copy of the form itself, not from a checklist a classmate passed along two years ago. The Office of Academic Advising and Student Services monitors compliance for graduate students and is the office to ask when a line on the form is unclear.

The same policy covers every BSN, MSN and DNP student, along with post-master's certificate students. RN-to-BSN students are the exception: their handbook asks for the immunization form only when a practicum site requests it.

The physical exam and its one-year window

Kent State asks for a physical exam completed within one year before you enter a clinical site. The examining provider confirms two points: that you can carry out clinical activities, with or without accommodation, and that you show no communicable disease. The catalog states the stakes plainly. Without that exam on record, Kent State may be unable to place you.

The window counts back from the day you start at a site, and that detail catches students in longer sequences. Kent State graduate concentrations run from two to five practicum courses, and a later course often brings a new agency. An exam that easily covered your first site can pass its one-year mark before a second or third agency starts you, so compare the exam date with each new start date, not only the first one.

Annual TB test and flu vaccine: the items that lapse

Most of your compliance file is finished once. The annual TB test and the flu vaccine behave differently: the MSN Student Handbook names them as items that can expire partway through a program. Its rule has no grace period. While any compliance item has lapsed you complete no practicum hours, and nothing counts again until the record is current.

Lost days come out of a fixed calendar. Every practicum hour has to fall before the semester's final day, so even a short pause mid-course crowds the closing weeks. If that is already happening, read behind on clinical hours and the semester rules.

Vaccine exemptions and what they can cost

The catalog speaks directly to students who cannot or will not be vaccinated. You contact the Office of Student Services when you are admitted, which puts the conversation at the very start of your program. Kent State is candid about the reason: some partner agencies may decline to host a student who is not vaccinated, and that can delay graduation.

An exemption leaves the rest of your file unchanged. What it changes is the set of agencies open to you. Kent State cites more than 750 clinical partners, and the catalog leaves the decision about an unvaccinated student with each agency, so the question belongs in the very first conversation with every prospective site, before a preceptor spends time on your paperwork.

Raise an exemption at admission, as the catalog directs. Waiting until an agency asks leaves the least time to find a site that will host you.

Where your health records go

Kent State sends compliance records to a third-party tracking vendor, and the catalog is plain that students pay all compliance costs. That covers the physical, titers, vaccines and the vendor account; the placement costs page sorts out who pays for what.

The BSN handbook names Castlebranch as the undergraduate vendor, tracking immunizations, physicals, CPR, insurance and background checks, and notes that records may be shared with clinical sites. No public graduate document names the graduate vendor, so this site calls it the college's tracking vendor; the college's student services office can tell you which account to open.

Health items an agency can add

Kent State's form sets the floor, and agencies can build on it. Kent State's university-wide affiliation agreement template, dated 2015, asks students to finish whatever background screening or drug testing a facility demands, and it also calls for a negative TB test within the past 12 months and HIPAA training. Your agency's current onboarding packet, not the old template, decides what it wants from you.

Can my employer's annual health screening count as the Kent State physical?

Kent State's catalog describes what the exam must show rather than where it happens: a physical within one year before you enter a clinical site, confirming you can do clinical work with or without accommodation and have no communicable disease, recorded on the college's form with a licensed provider's signature. If your employer's screening covers all of that, check with student services in Henderson Hall before you submit it.

What happens if my TB test expires in the middle of a practicum?

Your hours stop until it is renewed. The MSN Student Handbook bars practicum hours while any compliance item has lapsed and names the annual TB test as one that can run out mid-program. Renew before the date, confirm the new result shows as current with the tracking vendor, and if a scheduled day is lost, report it with a makeup plan to your preceptor and practicum faculty.

Can I get a vaccine exemption for Kent State nursing clinicals?

Kent State's catalog asks students who cannot or will not be vaccinated to raise it with the Office of Student Services as soon as they are admitted. Kent State names Castlebranch only for the BSN, in the BSN handbook. No public graduate document names the vendor that MSN, DNP and certificate students use; the catalog calls it a third-party tracking vendor.
