Preceptor and student evaluations in Kent State practicums

Kent State's graduate guidebook does not describe a midterm evaluation. Midterm and final preceptor evaluations are documented for undergraduate students only, on the undergraduate preceptor page. If your syllabus adds a midpoint checkpoint, the syllabus governs.
The preceptor's final evaluation
Faculty route the evaluation to your preceptor, who fills it in and submits it in Typhon. Preparation starts well before that: as each semester opens, the assigned faculty member sends your preceptor contact details, links, the syllabus and the Preceptor Guide to Typhon by email, so the tools are in hand from day one.
Two guidebook details are worth knowing. First, the evaluation becomes part of your record, but the guidebook says it does not set your grade; practicum faculty do that. Second, it labels the form an NP evaluation, even on the page aimed at CNS, Nurse Educator and administration preceptors. A preceptor outside the NP role may ask whether the form fits their work, and your practicum faculty are the right people to answer.
Your preceptor can see your schedule and logs in Typhon, so what you enter shapes what they rate. Logs entered inside the 15-day window give an accurate picture; a pile of entries made in the final week does not. The Typhon guide covers the logging rules.
Your evaluation of the preceptor and the site
You evaluate the preceptor and the site in Typhon as well. Write it as a record for faculty rather than a thank-you note. Be concrete about the patient mix you saw, how much independent practice you had, whether the setting matched the course objectives in the rotation description, and whether onboarding went the way the agency described it.
Faculty review evaluations as part of monitoring Typhon, so your comments reach the people who confirm placements. If something went seriously wrong, such as a safety concern or a preceptor who was rarely there, raise it with your practicum faculty during the term instead of saving it for the form.
A simple habit helps with both sides of evaluation: jot a few lines after each practicum day about what you did and what you learned. Those notes make case logs faster and your end-of-term evaluation more accurate.
How practicum faculty review your work
Practicum faculty act as the liaison between the College of Nursing and your preceptor. In Typhon they monitor your time logs and case logs, send out the preceptor evaluations and review what comes back. At semester end they also complete hour-verification forms for preceptors.
